Don't force family members to make your end-of-life decisions - fill out an advance directive at Gundersen Lutheran - Spring Grove Clinic.

The clinic has advance directive consultants who can help individuals with the process of coming to understand, reflect on, discuss and plan for a time when they cannot make their own medical decisions.

The process is crucial to assist those closest to a person and their physician(s) to provide the best care when an individual cannot make their own decisions.

Through this process participants can create a plan called an advance directive.

"The planning you are starting is basic planning. It is important for you to complete an advance directive while you are healthy so that it is there if your health declines," explains Robert Breitenbach, MD, Family Medicine physician at the Spring Grove Clinic.

"When you put your wishes for care in writing, you are taking the burden off your loved ones to guess what you would want done, if you became unable to speak for yourself."

To help patients begin the process, the Gundersen Lutheran - Spring Grove Clinic is offering free advance directive consultations on Wednesday, Dec. 19, beginning at 9 a.m.

"By bringing this service to Spring Grove, we hope patients will take the opportunity to have the important discussions about their healthcare and complete an advance directive," says Breitenbach.
